In order to obtain an official transcript, the following form must be filled out  and returned with payment to the registrar.  A transcript can only be sent after the request form below and payment and a copy of a valid photo ID to the registrar have been received.  Remember, official transcripts cannot be faxed but rather must be either picked up or mailed.
