Ms. Shalaya Lee, 708-825-3121, shleeflowers@auntmarthas.org. Ms. Lee will be housed at SSC, but is available for anyone in our D205 community. Please include Mr. Porter and Patricia A. Malopsy-Fortier on any correspondences with Ms. Lee so that we may assist in the process.
Attached below is the application that all participants requesting assistance must complete. This form is lengthy and some of our applicants may become frustrated with the process. If they need assistance, please have them reach out to your Social Workers and/or Outlook Academy and we will help them complete the forms.
